Offerors must:  |

- Complete the Open Ratings, Inc. past performance evaluation forms (See Attachment 2). Offerors are reminded that they are responsible for payment of $125.00 to Open Ratings, Inc.
- Include one copy of the completed evaluation forms, Attachment 2, with your offer to GSA, clearly showing the date of submission to Open Ratings, Inc.(should be submitted to Open Ratings as soon as possible, suggested to submit prior to submitting offer to GSA to assist in expediting the evaluation process).

If you experience difficulty producing a GSA schedule proposal with the GSA Wizard, we will deduct the amount paid for the product from the price of a professional services contract for producing your proposal for you.
